Month-to-Month Membership

Automatic Enrollment Pay

I hereby agree to the following:

• Monthly payments, will be charged to my card starting today and will follow on the same date as each following month for your Tuition.

• Credit card declined and/or unsettled balance.


1. a. If the credit card is declined, Florida Center For Capoeira will charge the card on file the following Friday/s and/or until payment is received. 


1. b. If the credit card is declined after 2nd attempt, the following will take place.

1. b.a. A $25 fee will be added to the balance.

1. b.b. The participant will receive an invoice with a 14-day grace period from the stated date.

1. C. Participants must pay their account balance prior to attending classes. An unpaid balance after the 14-day grace period may or may not lead to a collection claim.

• Stopping month-to-month:

If participants wish to stop auto payments, a 14-day written notice by email to info@tfc4c.com is required.

2.a. No Refunds. Payments are nonrefundable and there are no refunds or credits for missed classes. Following any cancellation, however, you will continue to have access to the service through the end of your current billing period.

• Missed classes will not be credited.

• Inappropriate behavior towards teachers, substitute teachers, instructors, guests and/or fellow participants will not be tolerated in, Capoeira Classes, Rodas (Circles), Fitness Classes, Programs, Workshops, Seminars, Retreats, Ceremonies, Annual Events, Capoeira Performances and/or Demonstrations. Florida Center For Capoeira, its instructors, and/or teachers reserve the right to suspend, revoke membership, and/or expel participants from the group, due to inappropriate behavior.


ACKNOWLEDGMENT

I acknowledge that I have read this agreement, that I have executed this agreement voluntarily, and that this agreement is to be binding upon myself, my heirs, executors, administrators, and representatives.
